The Unsane pic is real, the sad thing about it is that the guy in the picture had died accidentally as opposed to intentionally of his own will. Can't remember how I know this but read about it a couple of years ago. One of the members of the band worked in an insurance firm and would see countless pictures like this, making copies or stealing the ones which he particularly liked.
The thing about the album cover is that it's so real it doesn't look real, if you get me.
