That sleeve looks like a really, really bad knock-off of the
Dan Seagrave paintings
which used to adorn death metal albums on Roadrunner Records in the early 1990s.

Album title of the year?
It does remind me slightly of an Obituary (whose debut I loved) album or something similar;
To be honest the first act the cover art called to mind was eighties psychobillies The Meteors;
