In Slovakia it is Slovan Bratislava vs Spartak Trnava, even if Trnava have only won one league title since the early 1970s. As for off the field, either away fans are banned completely, or up to 500 police and private security hired hands are on duty to keep 50 or skinheads from either side apart. One of last season's meetings was marked by Slovan's chief exec doing a Nazi salute in front of his club's supporters. Never an occasion to be savoured.

The way the 'Big 12' are distributed (4 from São Paulo, 4 from Rio, 2 from Porto Alegre and 2 from Belo Horizonte) means intra-city clássicos occur with great regularity but, given the size of their supporter bases, the biggest 'national' clássico is probably Flamengo v Corinthians.

Swansea-Cardiff in English League Wales.
In Welsh League Wales the 'big one' is still to be determined. It will be TNS v someone, but nobody has built up a consistent rep as the main contender over a sustained period. Barry are aiming for it having once been the dominant club and hoping to reclaim former glories.

It used to be Basel vs Grasshopper. But both are very much trading on former glories, particularly Grasshopper. That they are no longer the obvious top two clubs in the country may well have taken some sting out of it.
I went to one of these games back in the day, and it was a properly spiteful atmosphere. To the point where the ref took the teams from the field at one point, and as they were heading down the tunnel a length of metal pipe was hurled from the stands at the players/officials. It must have been this close to bring abandoned.

Thanks about Humberto, current forecasts has it closest to the island at Cat 1 or 2 by 90 miles late Wednesday night, hopefully just a strong blow and lots of rain.
