                  These sound like a planning meeting at Channel 5. "Right lads (and token woman), we've got some titles with puns in, so get to work devising crappy shows to crowbar into the title. So, for Fetch-22 we're doing a reality dog training contest - we start with 22 adorable yet naughty mutts, and whittle them down over the series until only one remains! Come on the rest of you - I've done one and I didn't have to think! I want full series synopsisesesies, er, whatever, on my desk in the morning!"

                    At the time (over 20 years ago), I did an internet search to make sure no one had thought of it before. It wasn't a book title, but someone had used it as a headline on a cricket match report.

                    I sent the publisher a list of 10 possible book titles. FWTBR was at the bottom of the list, and my least favourite, but that was the one they wanted. I have no clue any more what the other titles were.

                    It's just occurred to me that the book was published exactly 20 years ago this week. Bloody hell.

                        Ha! Didn't much like that title either, but you can't rely on any fucker at a publishing house to help you out with creative ideas, so that's what we went with.
